Subject: 2025-26 Athletics Registration- Final Forms
Dear Upper Arlington High School Parents and Guardians,
As we wrap up the 2024–2025 school year and begin planning for 2025–2026, please register your child for any sports they are interested in participating in by June 8, 2025. Early registration helps our coaches gauge interest in their programs and stay connected with interested student-athletes and families, and ensures that medical information is on file ahead of summer workouts.
Here's what you need to do:
1. Register your high school student-athlete(s) via
FinalForms before June 8, 2025 for the sport(s) they want to
participate in during the 2025-2026 school year.
2. Upload an updated pre-participation physical form, if
applicable, for your student-athlete(s) via FinalForms.
Ready to get started? You can sign in to your existing account OR create an account and register your child now at https://upperarlingtoncity-oh.finalforms.com/. Simply follow the prompts by clicking on the blue button as seen below, add any new student(s) if necessary and sign the forms. If you need assistance, please check out the parent playbook. Once completed, your student-athlete will need to re-sign their forms as well.
